Firearms Applications and Instructions

If you are applying for a Firearms ID Card (F.I.D. Card) you will need to complete the below applications:

1)   S.T.S. 033  State of New Jersey Application for Firearms Purchaser Identification Card and/or Handgun Purchase Permit

2)   S.P. 66 Consent for Mental Health Records Search

3)   LBPD Firearms Applicant Questionnaire

*Note: This form is voluntary only, and is not required under N.J.A.C Title 13 Chapter 54

4)   LBPD Firearms Applicant Reference Letters (2)

Once you have all the above forms completed, please come into the Police Station for application review and processing.

You will need two forms of identification:

One form of ID must be your Driver’s License with a Long Branch street address (no PO Box addresses)

Additional forms of ID:  Firearms Identification Card (must be presented if applicant has an FID card), Social Security card, Birth Certificate, Passport, Vehicle Registration, Auto Insurance Card, Voters Registration Card, Utility Bill (must be within previous 60 days of application date), Bank Statement (must be with previous 30 days of application date), City/State ID, and/or Green Card

If you have never possessed a FID Card, you will be required to be fingerprinted and will be given instructions on where to report. 

If you possess a FID card, please bring that with you as well as your driver’s license.

Once the initial application is accepted you will receive instructions on obtaining finger prints (in order for application to be approved fingerprinting must be completed within 30 days of application.

Firearm Permit applicant instructions (Once you’ve received your SBI Number)

Once on the website click on the ON LINE FORM 212A, (a highlighted block located in the lower left side of the page).

Follow the prompts for demographic and payment information. Payment can be made with a credit or debit card.

Upon completion of the form you will receive an email confirmation & receipt that will include a confirmation number. IMPORTANT: Print out the confirmation page for your records and return a copy to us by either:

            $5.00 (Firearms Identification Card)

            $2.00 (for each permit/handgun being purchased)

            $18.00 – Money order only (payable to:  N.J.S.P. SBI)

            $ 2.00 – Each permit/handgun being purchased


City of Long Branch fees may be paid in cash, but if fees combined add up to $10.00 or more then it must be paid as a money order

It will take approximately 15-30 minutes to process each applicant. 

Payments must be made by Money Orders only.  (No Cash, No Checks and No Credit/Debit Cards) 

Taxi Applicants must be re-fingerprinted every other year.  (NO EXCEPTIONS)

o    Two Forms of Identification are needed (One form must be a picture ID) like a Passport/City ID/Drivers License.

o    Must be a resident of NJ for at least 1 year

Renewal License

o    $50.00 Processing Fee (money order only)

o    Abstract

o    Have a second form of ID along with your Taxi License

o    Physical By Personal Doctor, Notary Public Must Notarize 4th Page (list of in-town Notary Public's)

o    Two Character References (They cannot be related to applicant and cannot reside at the same address as the applicant). o    

o   Update Photo

o    Must have 3 yrs of Driving Experience

o    Notary Public must notarize 5th page  (list of in-town Notary Public's)

o    Take Photo

o    Contributors Case # (LBPD Reciept #) Issued and written on Finger Print Form.
